چکیده
Neuromuscular disorders and injuries such as cerebral palsy and stroke often result in foot-drop which can result in a person having great difficulty walking. Ankle foot orthoses (AFOs) or splints have been prescribed for many years now to limit the range of motion of the ankle, provide the patients with support and assist with rehabilitation. However the majority of AFOs require a long, labour-intensive manufacturing process which results in unacceptable waiting times for children that are rapidly growing and patients with varying conditions. This research proposes a new approach to AFO manufacturing that utilizes digital and additive manufacturing technologies to customise the fit and form to an individual. By implementing an interchangeable carbon fibre spring at the ankle joint the design will result in a stronger, more comfortable, more flexible AFO that can adaptively constrain ankle movement for various different activities. Three iterations of AFO design have been developed and tested to validate their efficacy. A custom machine has been designed and constructed in order to empirically test stiffness values for the AFO and allow for optimal AFO geometry based on input parameters. This machine has proven the structural integrity of the final AFO design. Progress has been made in automating parts of the design process which will significantly reduce labour requirements and hence manufacturing delay times. *Corresponding author: A.J. The effect of ankle foot orthosis stiffness on the energy cost of walking: a simulation study.
BACKGROUND In stroke and multiple sclerosis patients, gait is frequently hampered by a reduced ability to push-off with the ankle caused by weakness of the plantar-flexor muscles. To enhance ankle push-off and to decrease the high energy cost of walking, spring-like carbon-composite Ankle Foot Orthoses are frequently prescribed. However, it is unknown what Ankle Foot Orthoses stiffness should b...

OBJECTIVE To measure walking speed and energy cost in patients with prior stroke with and without a carbon composite ankle foot orthosis. DESIGN Within-group comparisons of 2 walking conditions. PARTICIPANTS Convenience sample of 10 hemiparetic patients with a stroke at least 6 months earlier (average age 52 years) habituated to a carbon composite ankle foot orthosis. METHODS Subjects wal...
متن کاملNeuromechanical adaptation to hopping with an elastic ankle-foot orthosis.
When humans hop or run on different surfaces, they adjust their effective leg stiffness to offset changes in surface stiffness. As a result, the overall stiffness of the leg-surface series combination remains independent of surface stiffness. The purpose of this study was to determine whether humans make a similar adjustment when springs are placed in parallel with the leg via a lower limb orth...
متن کاملThe effect of a powered ankle foot orthosis on walking in a stroke subject: a case study
[Purpose] Standing and walking are impaired in stroke patients. Therefore, assisted devices are required to restore their walking abilities. The ankle foot orthosis with an external powered source is a new type of orthosis. The aim of this study was to evaluate the performance of a powered ankle foot orthosis compared with unpowered orthoses in a stroke patient. [Subjects and Methods] A single ...
متن کاملDesign and Evaluation of an Articulated Ankle Foot Orthosis with Plantarflexion Resistance on the Gait: a Case Series of 2 Patients with Hemiplegia
Ankle-foot orthoses (AFOs) have been described to have positive effects on the gait biomechanics in stroke patients. The plantarflexion resistance of an AFO is considered important for hemiplegic patients, but the evidence is still limited. The purpose of this case series was to design and evaluate the immediate effect of an articulated AFO on kinematics and kinetics of lower-limb joints in str...
